Observed the installation of epoxy dowels into existing concrete using Simpson xp
epoxy, expiration 2019. Verified the hole depths and proper cleaning with a nylon
brush and compressed air prior to epoxy. Schedule on sheet S2.1 of the appr. plan.
Locations:
1) Anchor Bolts for new column Lines AA/B1.8 detail 51.5/7 Note#1 =
(1) 3/4" Threaded Rod with min 12" embed.
Mason's were observed for proper low lift procedures and techniques. Also observed
the placement and consolidation of grout.
Location: 1st FI. Elv Mech. Rm Walls With (1st two courses only) (1) grout box made.
Observed reinforcing steel for size, grade, lap, location and clearance per
schedule
and details of the approved plan. Note# 11 on sheet S2.1
Location: 1st Floor 8" Elv. Mech Room CMU Walls (1st two courses only)
1) With #5's Vert @ 16" o.c and #5's Horz @ 16" o.c, with (1) added #5 @ corners,
end of walls and openings.

Weld size and all completed weld. Observed (FCAW) of new L 3X4X3/8 ledge angle and stiff plate onto existing
Beams with 1/4+ fillets & PL 1/2X7 continues plate onto bottom flange of existing beams work per detail 5&6/S1.5.2
On going installation of 3/4 all thread A307 galvanize bolts onto existing concrete columns and beams with 6" embedment
And new 3/4 and 1/2 embedment plats for new canopy connections using hilti HIT-HY 200-R (ICC ESR 3187)
Work per detail 2/S3.1.1 in progress.

Foundation Elements Reviewed:
Momennt frame grade beam steel reinforcement, moment frame anchor bolts,
pad footing reinforcement, epoxy dowels to slab, cmu dowels at elevator pit.
The moment frame column base plate located at gridline AC, Al.1 was offset
from centerline of existing grade beam. Please refer to sketch SK -2 for field
repair sketch.
The post that occurs at gridline intersection B1, BEI needed to supported on a
new pad footing since the existing grade beam is offset from existing wall and
does not allow for full bearing surface. Please refer to sketch SK -1 for
proposed repair solution.
The above repair recommendations per details SKI and SK2 were observed
to have been addressed on 6/7/2018.
NO DEFICIENCIES OBSERVED.
