Address 315 A, 315 B, 315 C, and 315 D East Bay Avenue
Please be advised that after reviewing the subject application, your submittal has been
deemed incomplete and further information is required before we are able to proceed
with the application process.
The following documentation is required to complete the application:
1. Application
i. Please provide a letter of authorization from the Property Owner authorizing
Sepulveda Builders to sign on behalf of the Property Owner.
2. Unit Occupancy Table
i. Please update tenant information to include the start date of tenancy and the end
date of tenancy for all four units.
3. Mello Act Acknowledgement Form
i. Mello Act Acknowledgement Form signed by the manager of 315 E. Bay LLC.
Please provide a letter of authorization from the Property Owner authorizing
Manager to sign on behalf of the Property Owner.
Upon verification of completion, the application will be processed and given to the
Community Development Director for review. Should you have any questions regarding
