Please be advised that after reviewing the subject application, your submittal has been
deemed incomplete and further information is required before we are able to proceed
with the application process.
The following documentation is required to complete the application:
1. PROJECT DESCRIPTION
Please submit a written project description that describes the operational
characteristics of the business and the suite.
Include details such as:
The square footage of the suite and number of stories, the hours of operation for
Saturday, the number of employees, the number of patients to be treated Monday-
Friday, the number of patients to be treated on Saturday, approximately how long
patients will remain onsite, etc., and describe the customer experience.
2. PLANS
Both the title sheet and the condominium map suggest the suite is two stories
however the provided floor plan is only for the first story. Please review and revise
as necessary.
3. APPLICATION
The application must be signed by the owner of the property or an authorized
representative. Please resubmit the application with the signature of the property
owner (the provided Title Report lists the property owner as The Jetty Newport
Beach, LLC).
4. MISCELLANEOUS
