The following documentation is required to complete the application: 1. PROJECT DESCRIPTION Please submit a written project description that describes the operational characteristics of the business and the suite. Include details such as: The square footage of the suite and number of stories, the hours of operation for Saturday, the number of employees, the number of patients to be treated Monday- Friday, the number of patients to be treated on Saturday, approximately how long patients will remain onsite, etc., and describe the customer experience. Partially addressed. Please update project description to confirm the gross square footage of suite B1. Title sheet of plan set lists an occupancy load of 2,721 square feet however staff requires the gross square footage of the suite to be specifically called out for calculating parking demand. 2. PLANS Both the title sheet and the condominium map suggest the suite is two stories however the provided floor plan is only for the first story. Please review and revise as necessary. Partially addressed. Suite B1 is shown in the provided Condo Plan as two floors. Project description states 2nd floor will be sold to a separate party and is not a part of scope. Parcel Map No. 89-128 does not provide information on how airspace can be divided. APPLICATION The application must be signed by the owner of the property or an authorized representative. Please resubmit the application with the signature of the property owner (the provided Title Report lists the property owner as The Jetty Newport Beach, LLC). Not addressed. Alternatively, provide a letter from an executive at The Jetty authorizing signature from Matt Hauge. Upon verification of completion, the application will be processed and scheduled for a Zoning Administrator Hearing.
