Nancy Soo Subject: Final Compaction Report for Hoag Hospital Ancillary Landscape Renovation, Located at One Hoag Drive, Newport Beach, Califomia. Ladies & Gentleman: Smith -Emery Geoservices is pleased to present this compaction report on the monitoring and testing of fill placement at the subject site using on -site and imported materials. Our professional services reported herein include work completed within the period between January 5, 2011 and April 11, 2011. Hoag Hospital Ancillary Landscape renovation consisted of installation of stormdrains, waterlines, electrical lines, curbs and gutters, hardscape, concrete walkways, entrance walkway, drop off and valet areas and valet parking. Our observations. grading and compaction testing were performed in accordance with the recommendations of this office and applicable Building Codes. 791 EAST WASHINGTON BOULEVARD LOS ANGELES, CALIFORNIA 90021 PHONE 213/745-5333 FAX 213/746-0744 Copyright © 2011 Smith -Emery GeoServices All Rights reserved. 1195 NORTH TUSTIN AVENUE ANAHEIM, CALIFORNIA 92807 PHONE 714/238-6133 FAX 714/238-6144 SMITH -EMERY GEOSERVICES Hoag Hospital Landscape Renovation One Hoag Drive Newport Beach, California EARTHWORK MATERIALS SEG File No. 39807-1-1 Report No.G-11-8495 June 9, 2011 Material used in the compacted fill consisted of imported on -site clayey sand and imported crushed aggregate base (CAB). FILL PLACEMENT Fill materials used in the compaction were placed in loose six- to eight -inch thick lifts, moisture conditioned at or above optimum moisture content, and compacted to at least 90 percent for on -site soils and 95 percent for CAB of the laboratory maximum dry density as determined by ASTM D1557-09 (five layer method). Earthwork equipments used for grading work consisted of vibratory equipments, gas -powered hand tamper (whacker) and a water hose/water truck. TEST PROCEDURES FIELD TESTING Density tests were taken in accordance with ASTM D6938-10, the nuclear gauge method. Approximate locations of the in -place density tests are shown on Plate A and the compaction test results are summarized in Table 1. 2 SMITH -EMERY GEOSERVICES Hoag Hospital Landscape Renovation One Hoag Drive Newport Beach, California LABORATORY COMPACTION TESTS SEG File No. 39807-1-1 Report No. G-11-8495 June 9,2011 The maximum dry density at optimum moisture content of each fill material used was determined in accordance with ASTM D1557-09 (five -layer method). Test result obtained for each soil type used is summarized in Table 2. CONCLUSIONS AND RECOMMENDATIONS GENERAL Based on our grading observations and field and laboratory testing, it is our opinion that all fill placement and compaction testing were at least 90 percent for on -site soils and 95 percent for CAB of the laboratory maximum dry density. Consequently, the compacted fills placed, will provide adequate subgrade supports for their intended use. DRAINAGE The site should be finish -graded to provide for positive drainage away from compacted area in accordance with the building code and applicable local requirements. Unpaved areas should slope no less than two percent to drain. Paved areas should slope at least one percent to drain. All concentrated surface drainage on -site should be collected in engineered, non -erosive drainage devices and safely conducted to the street. Due to the size, longevity and complexity of this multi -phased project we are requesting an extension of time. As discussed this project duration is approximately 44 months total and, OSPHD required the original city permit as a condition of approval in June of 2007. The project is mandated and required to have an assigned full time TOR inspection, along with a designated OSPHD area compliance officer, and district structural engineers that visit the site weekly. For this project we also have an independent testing and inspection firm which we have contracted with Kleinfelder West Inc. Attached are copies of the ongoing inspection reports for your backup as requested. Your consideration to this matter is greatly appreciated. Please feel free to call me with any questions. Walked the project, reviewed the progress, changes and compliance with Architect and Engineer. Construction Progress: Demolish existing steel beams and deck. Erect steel columns and beams. Deficiencies/comments: 1. Braces for roof top utility support frames shall be done prior to frames loaded. 2, Existing steel beams can not be reused if they are damaged during demo. SEOR has to figure out repair plan and submit to OSHPD for review. Repair process has to be inspected. New steal beams with same or higher grade can be used to replace existing damaged beams. 3. Any welding to structural members shall have approved welding procedure (WPS). Means and methods for connections to structural members shall follow WPS and connections shall be approved by SEOR. In any cases, structural strength shall not be reduced. 4. SEOR needs to verify (prove with original calculations) that beam W12x26 be able to carry unbalance Toad in special brace frame. 5. SEOR needs to verify (prove with original calculations) that lateral brace of moment frame column is strong enough with two bolts connected to beam.
