(Kleinfelder) provided geotechnical observation and field density testing services during earthwork activities for the referenced project. This report summarizes our geotechnical observations and the results of the in situ density tests performed during the grading operations for the Proposed Addition to James Irvine Surgery Center at Hoag Memorial Hospital Presbyterian. The location of the subject building pad is shown on Plate 1, Plot Plan. The field observations and density testing covered by this report were performed by representatives of our firm on a part-time to full-time basis between April 20, 2005 and July 23, 2005. C:\ 9lr11 41(n December 15, 2005 nl,C �nPi Wi,-I :)a;.n PROJECT DESCRIPTION The proposed addition to James Irvine Surgery Center included an approximately 800 square feet one-story wood frame structure with a slab on grade floor. The estimated maximum wall Toads are on the order of 1,000 pounds per foot. The proposed addition is on the west side of the existing Hoag Memorial Hospital James Irvine Surgery Center. MATERIALS/PROCEDURES Geotechnical observation and field density/moisture tests of the engineered fill materials were performed on a full-time basis during the precise grading operations and on a part- time basis during associated trench backfilling. Field density testing was conducted in general accordance with ASTM test procedures D 2922/D 3017 (Nuclear Method for Determination of Density and Moisture). The results of tests performed at locations selected by our on -site technician during construction are presented in Table 1, Compaction Test Results. Laboratory maximum dry density and optimum moisture content of the on -site and import fill materials were conducted to evaluate the compaction characteristics of the soils. The laboratory maximum dry density and optimum moisture tests were conducted in general accordance with ASTM Standard Test Method D-1557 (Laboratory Compaction Characteristics of Soils Using Modified Effort). The results of these tests are presented in Table 2, Maximum Unit Weight - Optimum Moisture Content Test Results. GRADING OPERATIONS The grading contractor utilized the following equipment during precise grading and backfilling: • Hand Operated Compaction Equipment • Backhoe with Sheepsfoot Roller Prior to placement of engineered fill, existing fill within the building pad was overexcavated to depths of on the order of 2 to 3 %z feet below finished pad grade. The overexcavation extended laterally up to 5 feet except along the north side of the pad where existing hardscape was designated to be left in place. The materials adjacent to the sidewalk were evaluated by our engineer to be suitable for support of the building. Fill soils were placed in lifts not exceeding 8-inches, moisture conditioned, and compacted by rolling with heavy equipment to a minimum of 90 percent of the applicable maximum dry density as determined in accordance with ASTM: D 1557-91. Fill soils consisting of on -site silty sand were placed and compacted to 90 percent relative compaction in the building pad area. Periodic compaction tests were performed to evaluate the conformance of the grading operations to project specifications. Areas in which density testing indicated relative compaction below project specifications were 1) scarified, recompacted and retested until minimum relative compaction criteria were met or 2) removed and replaced with materials that had a moisture content near the optimum moisture content to facilitate compaction. 54760/DBA5L206 Page 2 of 3 December 15, 2005 Copyright 2005 Kleinfelder, Inc. In our professional opinion, based on observations and test results, the earthwork observed was conducted in general accordance with the referenced geotechnical engineering report and the project plans. The field density test results at the locations tested are within the specified design parameters. Where testing indicated relative compaction less than the specked design parameters, the areas were reworked and retested. The fill soils observed and tested by Kleinfelder have been placed and compacted in accordance with the requirements of the referenced geotechnical report. The engineered fill discussed herein is considered to be suitable for support of the proposed building addition. LIMITATIONS The services provided as described in this report include the field density testing for the precise grading of the Proposed Addition to James Irvine Surgery Center as shown on Plate 1. These services have been performed according to generally accepted geotechnical engineering practices that exist in the area at this time. No warranty is provided, expressed or implied.
