Dear Mr. Jurdi:
Hoag has received and reviewed a copy of the concrete test results for the East Parking Structure, and we understand that
some of these tests have not achieved the specified strength at 28 days. We have also reviewed the structural design with
the project structural engineer of record and I acknowledged his May 23, 2001 letter to the City of Newport Beach that
addresses design modifications to accommodate the low test results.
The construction documents revised on May 22, 2001 that include the new concrete strength specifications, were reviewed
and we concur and agree that the recommendation is appropriate and these changes are acceptable to Hoag Hospital. If the
new design and supporting calculations comply with the Uniform Building Code, we request that the City approve the
changes.
On behalf of Hoag Hospital, I request that the City release their hold on concrete placement to allow the contractor to
continue the work as directed by the structural engineer. Thank you for your consideration.

Per today's request, please find attached approved copies of the current and revised concrete mix
designs for 6,000 PSI non -colored walls and columns.
Please contact me at your earliest opportunity to confirm McCarthy can resume placing concrete for all
areas of the project. We also request your assistance in scheduling Mr. Lloyd Dick so that he can
complete the deck inspection today so that concrete placing can begin tomorrow at 7:00 AM.
We have placed a inspection call for 5/25/01 at 8:00 AM for the colored shear walls and columns with
the anticipation of placing concrete in those colored walls on 5/26/01.

I have reviewed the attached United Concrete's Mix Desi 'Atl®l 5575, a 6000 psi, 1-inch maximum size
aggregate, pump -place mix proposed for use in concwalls at the above project per direction of the
Project Structural Engineer Ed Gharibans.
This is a new concrete mix design,and its performance record has not been established. Therefore, in order
to validate the projected 28-day strength Indicated In the mix design, the supplier allowed us to obtain 8
cylinder test samples for testing purposes from a jobsite test panel that the contractor poured on 05/12/00.
Subsequently, two of the eight samples we tested at 7 days (05/19/00) yielded an average compressive
strength of 5240 psL This result closely agreed with the 7-day strength of 5200 psi indicated in the mix
design. On 06/09/00, we tested another four (4) samples that showed an average 28-day strength of 7460
psi which is 13% better than the projected 6600 psi strength. We are retaining the remainder samples for
the optional 56-day test.
On the basis of the above result, the subject mix design appears to be satisfactory for the strength indicated.
We have also found the mix design to be In general conformance with the materials, aggregate, and
proportioning requirements for concrete under Section 1905 of the 1998 Uniform Building Code. We therefore
RECOMMEND THIS MIX DESIGN TO BE USED on this project, as designed and without any mater!a!
substitution.

We have reviewed the concrete strength test results provided by Teratest Testing
Laboratory. We noticed that the concrete strength came out lower than the prescribed
design strength on the drawings at various locations.
We have prepared calculations for several elements with the low strength and found it
to be structurally adequate. We have submitted the general note sheet indicating low
strength for various elements. We also provided a procedure to follow in the event the
design strength is not achieved in future concrete placement.
We are recommending to the contractor at this time to use a separate concrete mix
design with available historical data for all interior columns and shear walls- Usa the
mix design presently being used for all exterior columns and shear walls. In the event
the design strength is not achieved in the 28 days test, the procedure as outlined in the
revised general note sheet is to be followed.
The content of this report has been discussed with the owner's representative (Leif N.
Thompson)and they do not have any objection to this.

Remarks:
As per our discussion in the meeting with - Hoag Memorial, McCarthy. Taylor & Associates,
Teratest, & others, we are providing the following procedure to fallow in the event concrete strength
do not reach to the design strength in 26 days as indicated in the drawings.
• If there are extra cylinders, test them after 56 days. If the test result come up to or above the
design strength. concrete is acceptable es placed.
• If there are no extra cylinders, provide core testing or test by other approved methods. If
these tests indicate the strength of concrete is equal or better than prescribed designed
strength, concrete is acceptable as placed.
• If in either instance the tests indicate the concrete strength lower than the indicated strength,
we will prepare calculation if necessary for building departments review and approval
