Dear Ms. Lerch,
As requested, we have completed this peer review of the above referenced
second revision of methane mitigation plans for the proposed Hoag Hospital
Advanced Technology Pavilion retrofit. Initial revisions to the plans were made in
response to our original peer review comments dated June 5, 2008. A response
letter dated June 13, 2008 from Environmental Safety & Management Associates
(ESMA) and a letter from Terra Petra dated June 12, 2008 letter certifying that
the replacement membrane will be compatible with the existing membrane was
provided at that time.
The second revision to the plans was made in response to our second peer
review comments dated June 18, 2008. A response letter dated June 19, 2008
from Environmental Safety & Management Associates (ESMA) was provided
along with electronic PDF's of the changed sheets (G-02, G-09 & G-11).
Based on the information provided in the letter and the PDF's, the plan set
appears sufficient to construct appropriate methane mitigation measures for the
retrofit of the Advanced Technology Pavilion. Please note that as per any
construction project, coordination with the engineer of record will be required as
questions arise regarding specific provisions of the plan set.

The temporary gas detection system has been installed throughout the facility
and will monitor the building 24/7. There is alarni annunciation (Horn/Strobe)
available for all workshifts. All of the contractors have attended a safety
orientation for the Emergency Action Plan. The contractor is requesting to start
Saw Cutting asap. Please advise.

Six- (6) Pages (Including cover page).
1. The plan has been revised to include the appropriate references to the 2007
Codes for Los Angeles County and the City of Newport Beach on pages lthru 4
of the Gas Mitigation Plan.
2. All the plan sheets have been wet stamped by our Licensed Civil Engineer.
This plan has gone out for bid to qualified contractors with a C-10 and/or C-16
license. Once the contractor is selected they will proceed to the City of Newport
Beach to submit their licenses and to notify the City of Newport Beach that they
are ready to proceed with their scope of work. The electrical scope of work is at
least four to six weeks away. We hope this will be acceptable to your office.
3. Sheets G-07 has been identified as "EXISTING and "FOR REFERENCE
ONLY".
4. We have attached the UL approval certificate for the specified Gas Detection
System to this response letter and Page 10 of 11, of the Gas Mitigation Plan,

There is a fixed Gas Detection Systems installed throughout the Hoag Memorial Hospital ATP Building.
The ATP building has two- (2) Sentry — Model 5000 Controllers, ten- (10) Combustible Sensors, 0-100%of the
Lower Explosive Limit (L E L ), and two- (2) audible/visual (Horn/Strobe) annunciation stations.
The system is programmed to detect Methane at the Low Alarm (15% L.E.L.) and at the High Alarm (25% L.E.L.)
levels. The Gas Detection System panels correspond to a series of remote Gas Detection Sensors located throughout
the ATP building as illustrated on the Gas Detection Mitigation Design Plan Site Map, Page 7 of 11, and at the
controller location. The Site Foreman will have a Fire Extinguisher and a Portable Exhaust Fan onsite for corrective
measures.
A copy of the Emergency Evacuation Plan is located at the Site Foreman's station, Hoag Hospital PBX station, Hoag
Hospital Electrical Engineering office, and the City of Newport Beach Fire Department office.
Low Alarm Activation Procedure
If the Low -Level Methane Alarm (15% LEL) and the Amber Strobe is activated:
1. The Site Foreman will proceed to the Controller location and identify the alarm location, investigate the alarm
area, and phone the Gas Detection Service Company to assess the alarm condition and implement corrective
measures (Reset the alarm, Ventilate the area, notify onsite personnel of corrective action). The facility should
not be evacuated for this alarm condition.
2. if the alarm can not be corrected, the Gas Detection Service Company will proceed to the location, investigate
the alarm condition, perform a system service, re -calibrate the sensor(s), and reset the Gas Detection System to
the normal operate mode.
4. The Site Foreman will prepare and submit an Incident Investigation Report to the Safety Officer within 24 hours
of the event. The Gas Detection Service Company will prepare and submit a System Configuration Worksheet
to both parties within 72 hours. No further action will be taken.

High Alarm (Gas) Activation Procedure
If the High -Level Methane Alarm (25% LEL) and the Red Strobe/Horn are activated:
The Site Foreman or Alternate shall initiate emergency evacuation procedures for all persons in the ATP
Building area. The Site Foreman shall initiate emergency evacuation procedures for all persons to the
designated Evacuation Assembly Area, as illustrated in the Site Map at the Site Foreman's station. The Site
Foreman will take all onsite personnel to the Evacuation Area as required until the incident has been cleared.
The route in and around the ATP Building facility entrance must be kept clear for emergency service vehicles.
The Site Foreman shall take the EAP to the Evacuation Area and take Roll Call, call the personnel on the
Emergency Phone List, and assign support personnel to implement the EAP procedures.
2. The Site Foreman will contact the Fire Department and the personnel on the Emergency Phone List until they
acknowledge.
3. A support team member should wait for the Fire Department to arrive at the ATP Building Parking Lot area to
direct them to the incident location.
4. The Fire Department and the Site Foreman will proceed to the Controller location to identify and evaluate the
alarm condition.
Please note the following information prior to the Fire Department arrival:
a. The Site Foreman and/or authorized persons name;
b. That all personnel been accounted for and evacuated to the Emergency Evacuation Assembly Area;
c. Location and address of incident;
d. Time of incident;
e. Identify any injured persons that you saw or heard.
Note: Give this information to the Fire Department so they can implement the appropriate response procedures.
When the Fire Department arrives at the site, they shall manage the emergency from that time forward.
The Site Foreman will provide technical assistance, site control, and access to the facility for the Fire
Department.
Once the gas level has dropped below the High Alarm Level (25% L.E.L.), the Site Foreman shall perform the
following procedure on the Gas Detection System:
a. Press the Reset Button on the Gas Detection System— Once.
b. Press the Enter Button Once to Reset the system.

5. After the Fire Department con£ums that the building is safe to enter, the Site Foreman will notify all personnel
they may re-enter the facility.
6. The Gas Detection Service Company will proceed to the alarm location, investigate the alarm condition, perform
a system service, re -calibrate the sensor(s), and reset the Gas Detection System to the normal operate mode.
7. The Site Foreman will initiate the procedure to resume facility operations.
8. The Site Foreman will prepare and submit an Incident Investigation Report to the Safety Officer within 24 hours
of the event. The Gas Detection Service Company will prepare and submit a System Configuration Worksheet
Report within 72 hours. The Site Foreman will follow up with a Post Incident meeting to evaluate the incident
and any corrective measures as required.
Trouble Alarm Activation Procedure
1. The Site Foreman will contact the Gas Detection Service Company until they acknowledge. The Site Foreman
will not contact the Fire Department for Trouble Alarms.
2. If the Trouble alarm is sounding, the Site Foreman will review information on the Controller and call the Gas
Detection Service Company. The Gas Detection Service Company will proceed to the facility, investigate the
alarm condition, perform a system service, re -calibrate the sensor(s), and reset the Gas Detection System to the
normal operate mode. A System Configuration Worksheet Report shall be submitted to the Site Foreman. No
further steps need be taken.
