La Palma Ave Anaheim, CA 92806 Attn: Masood Mian Application No. Minor Use Permit No. UP2019-039 (PA2019-122) Address 100 W Coast Hwy Suite #105 Please be advised that after reviewing the subject application, your submittal has been deemed incomplete and further information is required before we are able to proceed with the application process. The following documentation is required to complete the application: 1. PROJECT DESCRIPTION Please prepare a narrative that describes, in detail, the proposed operation of the juice bar. For example, describe: - Hours of operation - Required parking for the use versus parking provided on site - Net public area (NPA) of the suite - Description of operations i.e. how will customers enter the suite, how will they order, where will they wait, where will they consume their product, how will the products be prepared - Description of what products will be sold i.e. only juice or will food be available as well - What will be stored on the storage racks - How will juice bar interact with Xanadu Café (as plans reference patrons using Xanadu facilities such as restroom) 2. PLANS - Site plan is not legible and it is unclear where suite is located on the property. Please provide a larger, legible, site plan and highlight specifically where the juice bar will be located. Label property lines. - Update address in project information block to include the suite number. - Please provide anticipated food and/or beverage menus or describe what will be sold in the project description. APPLICATION REQUIREMENTS No title report submitted in project application - One (1) copy of a Preliminary Title Report not more than six (6) months old from the date the application was submitted that identifies the legal description of property. 4. MISCELLANIOUS - Please see project review comments from the building department (including electrical/mechanical/plumbing) Upon verification of completion, the application will be processed and scheduled for a Zoning Administrator Hearing.
