The following documentation is required to complete the application: 1. Agreement a. Please note, a bond is required prior to construction. b. Please note, a master license agreement amendment is required prior to construction in order to identify the specific site locations approved under the agreement. c. Please note, an encroachment permit is required prior to construction. 2. SCE Letter Provide a letter from Southern California Edison authorizing the power supply for this site location. Provide the SCE power location and approved drawings from SCE as well. 3. Public Noticing Materials a. Only one set of mailing labels was submitted in application. Please provide a duplicate set. 4. Plans a. Project site is “Streetlight in public right-of-way, approximately 550 feet north of 1600 Jamboree Road (SLC2629)” b. See comments and redlines for corrections. 5. Alternatives Analysis I:\Users\PLN\Shared\PA's\PAs - 2021\PA2021-071\PA2021-071_20220406_INCOMPLETE.docx Page 2 Remove references from alternative site discussions that the facility is “stealth” (Class 1) under NBMC 20.49.030. Instead, the project is a screened right of way installation (Class 3). 6. Electronic copy All of the above submittal items are required to be submitted in electronic format (i.e., CD-ROM or PDF via email). 7. Miscellaneous Response letter provided in current submittal appears to be for a different cell site. 8. Pending Project Review Request Comments Your project has been routed to the Public Works department, Utilities department, and Real Property division for review. Comments and redlines from both Public Works and Utilities are attached. Comments and redlines from Real Property will be provided to you electronically as soon as they become available. Upon verification of completion, the application will be processed and scheduled for a Zoning Administrator Hearing. Per Council Policy L-16, all banner brackets on streetlight poles shall be mounted at least fourteen feet above the ground surface. Please revise this on the plans. 04/05/2022 – Not addressed. 5. Please dimension the existing and new distances from the curb face to the pole and cabinet. Ensure 18-inches from the curb face to the new cabinet. 6. Show existing streetlight conduits. 7.
