Dear Mr. Angelo:
Pursuant to your request dated May 1, 2013 (attached), we are assigning the address
1500 West Ocean Front for the new unit located above the structures addressed 101
and 103 15th Street.
We will advise City departments; however, it is your responsibility to inform all other
affected agencies, such as the U. S. Postal Service, Orange County Assessor, utilities,
etc. Please be sure to notify your local telephone service carrier so that responses to
any “9-1-1” emergency calls are dispatched to a valid address. The address numbers
should be at least four inches high, of contrasting color to their background, and clearly visible from the street.

I would like to request a new address for my unit. My first choice is 1500 W. Oceanfront. If that is not
possible, my second choice would be 1502 W. Oceanfront.
I’ve enclosed a copy of the Grant Deed indicating I am the Trustee of the property.
