completion of the structural system. Structural observation does not include or waive the responsibility for
the inspections required by Section 110 1704 or other sections of the Code.

I DECLARE THAT THE FOLLOWING ARE TRUE TO THE BEST OF MY KNOWLEDGE:
1. I am the engineer or architect retained by the owner to be in responsible charge for
the structural observation in accordance with the requirements of the City of
Newport Beach and in accordance with the Statement of Special Inspection (SSI)
outlining the required special inspections as required under section 1704.3 of the
2019 CBC. Provide a Statement of Structural Observation (SSO) If required by the
Architect or Engineer of Record.
2. I, or another Engineer or Architect who I have designated above and is under my
responsible charge, has performed the required site visits at each significant
construction stage to verify if the structure is in general conformance with approved
plans and specifications;

Observed the installation of four 5/8" all -thread anchor rods with min 8"
embedment into the existing footings at the rear of the residence for hdu2 hold
downs per detail 8/S-3.0. I verified that all holes were thoroughly cleaned with
a wire brush and compressed air prior to placement of epoxy. Simpson Set XP
epoxy was used. Epoxy expiration date was 8-10-25. Installation is complete.
