A state and federally mandated program
whose mission is to:
• promote adult empowerment
• prevent abuse
• advocate for rights & dignity of elderYy
and dependent adults living in long-term
care facilities

HUD requires that each organization providing services to
individuals with CDBG public service grant funds document:
- single female head of household status
- race
- ethnicity
- income (Income documentation is not required for "presumed
beneficiary" category clients) Presumed beneficiaries
include:.... a a a a a a 62 years of age) The intake
form includes age of resident.
A client intake form is completed for each investigation or each
Advanced Health Care Directive completed. This form requests
information on payment source. Man residents are on
SSI
(Supplemental SeuritY Income) which automatically places
them in
low income status.

Newport Beach's 15 Long -Term Care facHities house over
825 elderly and dependent adults. All these residents will
benefit by Ombudsman advocacy even if not personally
counted statisticallyas unduplicated. With full funding,
your Ombudsman will make over 150 unannounced facility
Yosft in the fiscal year 2016-2017 to accomplish the above.
