TITLE: Dolphin and Tamura Shallow Water Well Rehabilitations - Notice of
Completion for Contract No. 5981 (Project No. 15W11)
ABSTRACT:
On July 14, 2015, City Council awarded Contract No. 5981 (15W11), Water Well
Rehabilitation — Dolphin and Tamura Shallow Wells project, to General Pump Company
for a total contract cost of $276,280.00 with a $55,200.00 allowance for contingencies.
The required work to rehabilitate these two shallow water wells is now complete and
staff requests City Council acceptance and contract close out.
RECOMMENDATION:
a) Accept the completed work and authorize the City Clerk to file a Notice of
Completion for the project (At the July 14, 2015 meeting, this project was found to be
exempt from the California Environmental Quality Act (CEQA) pursuant to Section
15302);
b) Authorize the City Clerk to release the Labor and Materials Bond 65 days after the
Notice of Completion has been recorded in accordance with applicable portions of
the Civil Code; and
c) Release the Faithful Performance Bond one year after the acceptance by the City
Council.

The City owns and operates two well sites located in the City of Fountain Valley. The
first well site is located on Dolphin Avenue and the second is adjacent to Tamura
Elementary school and the well sites are named respectively. Each well site has a deep
and shallow well that tap into different aquifers. These four wells provide about 75
percent of the City's total water and at a significantly lower price than imported water.
The wells undergo routine inspections with downwell videos and flow meters to check
the well screen louvers for sanding, clogging and biofilm growth. Maintenance is
performed approximately every eight years based on the video and flow meter results.
Just after the Notice to Proceed was issued last November to the contractor to start
rehabilitation at Dolphin and Tamura Shallow Water Wells, the City's Dolphin Deep Well
failed. To address this pending emergency, the water well Contractor was directed to
immediately focus on restoring this pump to service. Work on Dolphin Deep Well
included troubleshooting and the complete rebuild of the pump at the Contractor's shop.
This added approximately 30 working days to the project.
Work then started on Dolphin Shallow Well. The initial downwell video revealed a crack
in the well casing. The crack was repaired with a high strength steel patch. The City's
ground water geologist/consultant Richard Slade and Associates (RSA) then reviewed
the video and flow meter data and found Dolphin Shallow Well to be in stable and
productive condition after the patch was installed. RSA recommended delaying the
planned additional rehabilitation work of the well, which involves rigorous wire brushing
and air bursting, until our next scheduled maintenance in 2024. Scheduling and
installing the patch required an extension of 20 working days. Work on the Tamura
Shallow Well proceeded as originally planned.

At the conclusion of the project, the actual cost of bid items constructed was $69,380.00
less than the original bid amount as a result of the rehabilitation bid items that were not
performed as planned on Dolphin Shallow Well. In addition to the cost reductions
associated with reduced bid item work, two change orders totaling $27,654.99 were
approved as part of the contract. The change orders included the above mentioned
steel patch, upgrading pump bowls to stainless steel, top column flange replacement,
repair of broken bolts on the sole plate, a downwell video survey of Tamura Deep Well
and a field trim balance of Tamura Shallow motor. These contract adjustments resulted
in the final contract cost coming in approximately 15.1 percent below the original bid
amount.

The project was substantially completed as of May 31, 2016. Some miscellaneous
close-out items, operation and maintenance turnover, and post -substantial completion
activities have been finalized prior to this Notice of Completion action.
ENVIRONMENTAL REVIEW:
At the July 14, 2015 City Council meeting, the project was found to be exempt from the
California Environmental Quality Act (CEQA) pursuant to Section 15302(c)
(Reconstruction of existing facility involving negligible or no expansion of capacity) of
the CEQA Guidelines, California Code of Regulations, Title 14, Chapter 3, because it
has no potential to have a significant effect on the environment.
NOTICING:
The agenda item has been noticed according to the Brown Act (72 hours in advance of
the meeting at which the City Council considers the item).
