Lifestyle • l Uf~ Editor Matt Coker ... 642-4321, at. S61 Facelift ol. ·the .future improvement over old method T be facelift of the future, known as the oomposite f acclift, is available today. Of particular interest, it has become the first procedure to adequately address the problem of loose skin or nasolabial folds (the folds running from the nose along the corners or the mouth). In this operation, the skin, fot and muscle of the check arc lifted and repositioned in one piece and not separated as is done with the standard lJcelif~ technique. This results in a smooth, ''unpulled" appearance ,, h1ch was not previously possible. First conceived in Sweden, the composite facelift was fully deve loped 1n 1968 by Dr. Sam Hamra, a plastic and reconstructivc surgeon in [)all as. ... It is difficult to evaluate a facelift," Hamra said. "Any improvement " freq uently regarded as a good or acceptable result." I lamra added that the status of the nasolabial folds and the contours 01 the jawline and neckline need to bc.Lvaluated following surgery. The composi te facelift produces a result similar to what is seen when the skin of the cheeks is pulled-upward with the hands. In this procedure, the total release of skin, muscle and fa t together allows the tl:ip of the face to be lifted back to its original position. The tradi tional method does very little to smooth out the folds of the face and often appea rs mask-like. Developed in the 1920s, simple skin-lift surgery was the standard proci:dure used by plastic surgeons. Today, most utilize a technique ' which elevates the deep muscles of the (ace as a separate layer from the lkin. ........ . ...... This procedure. called SMAS (Superficial Muculo Aponcurotic System), was considered statc·of-the-art until 1hc introduction of Harnra's composite facelift. The skin-only procedure is considered a first-generation facelift, the SMAS lift is referred 10 as a second-generation technique and the third genera tion is the composi1c, the most medically advanced procedure to date. The composite lift involves incisions running from above the• car and extending Crom in ide the car canal ro behind the car and into the hairline of the neck. Another Incision ls placed underneath the chin .. Through these incisions, the skin is lifted approximately one inch. · Image lnfonnation At this point, the composite nap begins and the dissection goes through all layers of the face and extends to the mouth. The composite flap is then held in place in fro nt of the ears with dissolvable sutures. Beneath the chin, 1he neck muscles thal have split apart with age are folded back together with stitches. This eliminates the "turkey-gobbler" look. I For the first time, a facelift is capable of correcting the jowls without the irregularities associated with liposuction. Every patient's face has its own natural layer of fat, which is easily manipulated by the surgeon using the composite technique. In addition, the problems relating tu the bags of the checks and nasolabial folds have t'inolly been resolved. Another benefit of the composite technique involves the neck. The fat of the neck is not separated irom 1he skin, leaving a smoother, unpullcd effect. This fat is frequently trimmed to give a sculpted neckline. There is minimal pain following a composite (accllft. Many times, however, patients experience numbness of the checks (or four to six \\.eeks. In addition, the face becomes more swollen after this procedure than with 1hc standard facelift because the surgery is more involved and elevutcs deeper layers of the face . The results speak for themselves. Patients actually look younger because the fat and jowls arc lifted bnck to their original position. They appear very natural and youthful without the suggestion of surgery. 0 ver the ycurs following surgery, this facelift ages gracdully since it is based on the natural repositioning of the patient's anatomy. Dr. Hamra has introduced the composite faceliCt to nearly half of the country'~ 2,800 board certified plastic and reconstructivc surgeons. He estimates, however, that only 100 have mastered the technique. Dr. Bt nvcnutl Is a board-ccrt/Ocd pl.stk •ad nconstrvcll•·t SUl'lffD who has pnctlttd In Newport &ach for 10 )Tars. He Is a dlplomatt of tht American Board of Plastic and Reconstruttlve SutJtry, diploruaie of the A.merlcao Board of Sursery, fellow of the American Colltgt of Surrcons and tlinlcal instructor al UCl 's School of Mtdlcinc.
